چکیده

Current guidelines recommend using bivalirudin as an alternative to unfractionated heparin in patients with heparin-induced thrombocytopenia undergoing percutaneous coronary intervention (PCI). Pulmonary vein isolation (PVI) is a well-established treatment strategy for patients with drug-refractory atrial fibrillation (AF) and requires systemic anticoagulation to avoid thromboembolic events, generally achieved with intravenous unfractionated heparin. There are several reports in the literature of bivalirudin use in place of unfractionated heparin during PVI with radiofrequency (RF) energy in patients with heparin-induced thrombocytopenia. To our knowledge, this is the first report of the safe use of bivalirudin for PVI using a cryoballoon catheter.

Successful intraprocedural anticoagulation with bivalirudin during pulmonary vein isolation in a patient with known heparin-induced thrombocytopenia type II

We report the case of a 56-year-old female who presented with symptomatic paroxysmal atrial fibrillation. Anamnestic heparin-induced thrombocytopenia (HIT) type II was suspected, and a rapid diagnostic test showed antibodies against platelet factor 4. The heparin-induced platelet activation-assay was negative. Radiofrequency pulmonary vein isolation with intraprocedural anticoagulation using bi...

MEDAFI-Trial (Micro-embolization during ablation of atrial fibrillation): comparison of pulmonary vein isolation using cryoballoon technique vs. radiofrequency energy.

AIMS Cerebral embolism is a possible serious complication during catheter ablation of atrial fibrillation (AF). The purpose of this prospective pilot study was to analyse the incidence and possible impact of cryo ablation on cerebral lesions and possible differences to radiofrequency (RF) ablation during pulmonary vein isolation (PVI). METHODS AND RESULTS Pulmonary vein isolation was performe...

Successful cryoballoon pulmonary vein isolation in a patient with situs inversus and dextrocardia

A 79-year-old man with situs inversus and dextrocardia underwent catheter ablation of symptomatic paroxysmal atrial fibrillation. Pulmonary vein isolation (PVI), using second-generation cryoballoon under Ensite NavX system guidance, was performed successfully in a reverse manner, which required short procedure and fluoroscopy times, as required in a PVI performed on a normal heart without any c...
